Summary: Job responsibilities include promoting, maintaining, and
improving individual and community health by assisting individuals
and communities to adopt healthy behaviors and preventing the
spread of HIV. Perform testing, counseling, outreach, and linkage
to care for high-risk residents in the community. Ensure
implementation of timely disease intervention strategies to
decrease the prevalence of HIV and STDs. Collect and analyze data
to identify community needs prior to planning, implementing,
monitoring, and evaluating programs designed to encourage healthy
lifestyles, policies and environments. Other duties as assigned.
This is a grant-funded position under the HIV Prevention Program.
Essential functions include, but not limited to: Assesses community
health needs to focus HIV Prevention efforts on target populations.
Conducts community outreach, including HIV Counseling & Testing
events. Provides educational programs and in-service training
sessions as well as participation in community health fairs to
enhance public understanding of HIV/AIDS. Provides case management
services, i.e., linkage to care, patient education/counseling, and
partner services to clients with HIV. Prepares and disseminates
educational and informational materials. Develops and maintains
cooperation between public, civic, professional, and voluntary
agencies for partnership for testing events and condom/safe-sex kit
distribution. Performs HIV and STD reporting, surveillance, and
Minimum Qualifications: Master's degree in public health
education from an accredited college or university OR Bachelor's
degree in education, social, behavioral or biological sciences or a
directly related field from an accredited college or university AND
One year of experience in public health education.
Preferred Job-Related Skills: Experience with HIV prevention
activities, HIV Counseling & Testing certification, HIV and STD
surveillance activities. Excellent verbal and written communication

Salary Information: $ 34,255 annually / Pay Grade I
