a qualified Home Health RN in Marietta, Georgia, 30067! Job Title:
RN Home Health Profession: Registered Nurse Specialty: Post Acute
Care Duration: 13 weeks Shift: Day Hours per Shift: 8:30 AM - 5 PM
Experience: Minimum 1 year in Medical/Surgical Nursing required;
Home Health and ICU experience preferred License: Registered Nurse
License (single state or multi-state compact) Certifications: BLS -
Basic Life Support; Georgia Driver's License or valid state
driver's license; Private Vehicle Insurance Must-Have: Strong
critical care nursing skills; computer/data entry experience;
ability to communicate with various members of the healthcare team;
ability to use Microsoft Excel and Word; basic computer operational
knowledge; Epic and 3M experience preferred Description: The RN
Home Health is responsible for providing initial, resumption, and
recertification evaluations of patients in their home settings.
This role includes performing routine home health visits and
implementing age-appropriate interventions that address physical,
emotional, developmental, psychosocial, and educational needs. The
RN collaborates with physicians, patients, family members, and team
managers to develop and coordinate the patient care plan. Regular
communication with all team members, including RNs, LPNs,
Therapists, Social Workers, and Home Health Aides, is essential to
ensure appropriate development of the patient's plan of care.
Direct patient care involves assessing, coordinating, and managing
the patient's total needs while following physician orders and
utilizing the nursing process. The RN performs routine and complex
nursing procedures to meet patient needs. Coordination of services
during the episode of care requires collaboration with physicians,
patients, families, and team managers to develop care plans. The RN
maintains communication with the physician, reporting any changes
that may affect the patient's recovery and developing the care plan
communicated to Home Health Aides. Patient and caregiver education
is provided to encourage self-care and achieve positive outcomes.
Utilizing verbal and written tools facilitates learning and
promotes independence during recovery while ensuring any concerns
affecting safety are reported. The RN communicates patient status
changes and relevant lab results to physicians as needed and
participates in care team conferences while checking for voicemails
at least once daily. Demonstrating knowledge of CMS/state and
regulatory guidelines is essential. Accurate completion of required
documentation is required within established timeframes.
Participation in the orientation and education of new employees is
expected when appropriate. The RN must attend continuing education
and in-services as directed and necessary to maintain clinical
knowledge related to home health care. Support and participation in
performance improvement activities are crucial, including chart
reviews and system-wide committees. Identifying and implementing
methods for improving department performance and care is also part
of the role. Meeting established productivity standards,
participating in weekend and call rotations, and performing other
duties as assigned by leadership are expected. The RN complies with
