LinkJobs works with large health care systems, small clinics and everything in between, and we are currently searching for the right person for the following position:Home Visiting Nurse PractitionerResponsibilities: Manage the transitional and chronic care needs of homebound patients including:Assessment and Evaluation of current conditionTake past medical historyReview symptomsConduct general physical examinationsReview and reconcile medicationsScreen for depressionScreen for alcohol and drug abuseIdentify diagnoses to be used in care management and active medical management for purposes of treatmentRecommend appropriate measures, including medication prescriptions and skilled needs for the patientEfficiently communicate findings in your assessment to the office / supervising physicianEducate patients on topics such as disease process and prevention, medication, and complianceQualifications:Current NP license, with no sanctions, restrictions, or provisions, in the state of GeorgiaAt least 1 year of clinical experienceClinical background in adult, family or geriatric specialtiesExperience working with an Electronic health record (EHR)Effective communication skills are criticalEase of use with smartphone and email are requiredReliable transportationAbility to work independentlyHome care or home visiting experience (preferred)Willingness to spend at least 30 minutes to 1.5 hours with a patient in their homes, which may or may not be in the presence of pets, and/or family members that are tobacco users
