healthcare.Responsibilities The Executive Director of Hospital Care
Coordination works closely with the AVP, Care Coordination and
Directors of Hospital Care Coordination to develop tactics that are
aligned with current performance and vision for the care
coordination program, including compliance with all CMS
regulations, assuring timely patient care progression, appropriate
care and resource utilization, safe discharge planning as well as
synthesizing and apply data analytics to drive system care
coordination performance.The Executive Director is responsible for
drafting, disseminating, and monitoring adherence to Wellstar Care
Coordination policies and procedures and standards of care, in
accordance with CMS and payer regulations. The Executive Director
ensures that the Wellstar strategic vision is fully integrated into
the culture of hospital care coordination. The Executive Director
works closely with Care Coordination team members and Hospital
Physician leaders to align standard work of care coordination
across the system and service lines.The Executive Director of Care
Coordination is responsible for the day to day monitoring of system
wide care coordination performance, including patient engagement in
discharge assessment, efficient transition planning, avoidable day
reporting, and overall efficiency for care management
processes.This Executive Director facilitates collaborative
development of standardized care coordination processes with
hospital CC leaders. As part of the role, the Executive Director
will collaborate with physicians and other departmental leaders and
staff to ensure that barriers to care coordination outcomes such as
length of stay (LOS), avoidable days, readmissions, payer denials
and other care coordination related outcomes are addressed timely
with attention given to identifying root causes and driving
performance improvement initiatives that ensures performance is
aligned to targeted goals. Must be able to manage, interpret and
share data with Hospital Care Coordination leaders.Other duties of
the Executive Director include creating and enhancing current
tools, methodologies, recruiting practices, and personnel
development processes. Plays a key role in the professional
development of care coordination leaders and staff, including
mentoring and coaching.The Executive Director participates in
system committees to address and solve issues that drive variation
from goals. Additionally, the Executive Director is responsible for
collaborating with other key departments that drive throughput
delays and work to jointly eliminate those barriersJob
Requirements:Required Minimum Education: BSN graduate from an
accredited school of Nursing with a current RN license in the State
of Georgia AND a Masters Degree (MHA, MBA or MSN preferred),
ORMasters in Social Work from an accredited school with current SW
license in State of GeorgiaRequired Minimum
Licensure/Certification: Currently licensed as a Registered Nurse
in the State of Georgia or hold a privilege to practice in the
State of Georgia under the Enhanced Nurse Licensure Compact (eNLC)
or SW license in the state of Georgia; Current Certification in
Case Management, provided by either ACMA or CMSA.Required Minimum
Experience: 5 years management experience of an interdisciplinary
team in the field of hospital care coordination with at least 8
years of recent experience working in an acute care hospital
environment. Clinical practice experience with complex patient
populations is required Supervisory experience of large staff team
is preferred. Supervision of multiple hospitals, teams or programs
is preferred.Required Minimum Skills: Excellent organizational,
verbal, and written communication skills. Independent and
self-directed. Strong ability to analyze, synthesize. and present
data. Ability to handle and manage change, perform critical
analysis, and mentor and develop practice in matrix organizational
framework.Strong collegial leadership engagement skills.
